Water Service Connection

  1. Who may avail of the service?
    • A resident within the City of Dagupan, or any his/her authorized representative
    • At least 18 years of age
    • Agrees with the Rules and Regulations set by the Dagupan City Water District on Water Service Connection now enforced and may be enforced thereafter.
  2. Requirements
    • Name of Applicant
    • Location of the premises to be served (present / draw a sketch)
    • Whether the premises have previously been supplied with water by the District
    • Purpose of which service to be used (e.g. Residential or Commercial)
    • Community Tax Certificate (Current)
    • Ordinance # 165 (City Assessor’s Office)
    • Latest Real Property Tax Receipt (Xerox Copy)
    • Real Property Tax Declaration

How to avail of the service

Step 1 Secure from the New Accounts Counter an Application and Contract for Water Service Connection Form. Inquire from the personin-charge. Gives the Applicant the Forms and provides a short briefing on the services to be rendered and the requirements needed including the rules and regulations set by the Water District on water service connection. 5-10 minutes Fernando A. Step 2 Accomplish the said Application and Contract for Water Service Connection Form and submit the same to the person-incharge together with the requirements.
